How to use ADL testing services

To simplify the logistics of transport of Test Kits and referral of specimens from all over the United States, we have adopted the following simple options. If you are outside of the United States please contact your local ADL agency.

Practitioners

1) Practitioners can issue test kits and request slips to the patient from their clinic.

• Practitioners can order test kits, stock them in their clinic and issue them to patients as needed. Test kits can be ordered by contacting ADL Customer Service at (877) 983 7863.
• Age Diagnostic Laboratories will dispatch Test Kit(s) requested, free of charge, the next working business day.
• Once the test kit has been issued by the practitioner, the patient collects the sample(s) and pays ADL directly by filling out the credit card details on the request slip or by sending a cheque, credit card details or money order with the samples. The sample is returned to ADL through the mail using the pre-paid envelope provided inside the Test Kit.
• Specimens are processed upon arrival at ADL and results are forwarded to the referring practitioner via post, fax or through computer downloads.

2) Practitioners can fill out personalized request slip with required test(s) only and issue to patient.

• The patient can then call the toll free number 877 983 7863 to order and pay for their pre-paid Test Kit(s).
• Age Diagnostic Laboratories will send Test Kit(s) to the patient’s preferred address by mail, the next working business day.
• The patient then collects the specimen(s) and returns them to ADL through the mail using the pre-paid envelope provided inside the Test Kit.
• Specimens are processed upon arrival at ADL and results are forwarded to the referring practitioner via post, fax or through computer download.
• Practitioners can order personalized request slips by calling ADL Customer Service at (877) 983 7863.

Patients

• Patient’s may order self-requested tests by calling ADL at (877) 983 7863.
• ADL will send Test Kit(s) to the patient’s preferred address by mail, the next working business day.
• The patient collects the specimen(s) and returns them to ADL through the mail using the pre-paid envelope provided inside the Test Kit.
• Specimens are processed upon arrival at ADL and invoice for payment and test results are mailed to the patient once finalised.
• ADL strongly recommends that results from patient self-requested episodes, are discussed with a healthcare practitioner in order to establish necessary treatment and therapy if indicated.
